Copley Hospital has served Lamoille County and north-central Vermont since 1932, providing the highest quality of care. A 25-bed, not-for-profit critical access hospital, Copley provides a unique blend of quality, compassionate, personalized, state-of-the-art medical care in a small, warm, friendly environment. With more than 65 specialists, Copley provides a full range of both inpatient and outpatient care, including 24-hour emergency services and the Copley Birthing Center.
Northwestern Medical Center is a private, not-for-profit, 70-bed primary care hospital. Through careful strategic planning and continued community involvement and support, NMC continues to invest in the staff, physicians, equipment, information systems, and facilities necessary to provide exceptional care for their community.
Founded in 1904, Brattleboro Memorial Hospital is a licensed, 61-bed, not-for-profit community hospital located in southeastern Vermont, serving a rural population of about 55,000 people in 22 towns in Vermont, New Hampshire, and Massachusetts. The BMH medical staff includes board-certified providers in primary care and many other specialties, with a shared mission of providing exceptional health care delivered with compassion and respect.
Cottage Hospital has served the residents of the Upper Connecticut Valley for 120+ years. The 35-bed Critical Access hospital continues to be recognized for providing exemplary care. Cottage Hospital proudly offers cutting-edge services – a designated Trauma Center, a multi-specialty Rural Health Clinic, the Center of Orthopedic Excellence, and the Ray of Hope acute voluntary inpatient mental health unit.
Adirondack Health offers a full range of medical and surgical services, in addition to primary care at three health centers, and long term care at two skilled nursing and rehabilitation facilities. The main hospital is a 97-bed acute care facility located across from Lake Colby in the beautiful Adirondack Mountains of New York. AH is the largest health care provider in the six-million acre Adirondack Park, serving the residents of five counties. Our 60-member medical staff is board certified in 21 specialties.
Grace Cottage Family Health & Hospital is an independent, non-profit healthcare facility with a Federally certified Rural Health Clinic. Its 15 medical practitioners provide primary care, pediatrics, geriatrics, and mental health services. The hospital is a 19-bed inpatient facility with hospice suite. Grace Cottage also has a busy outpatient physical and occupational rehabilitation department and retail pharmacy.
Gifford includes three organizational entities: Gifford Health Care, Gifford Medical Center, and Gifford Retirement Community. Its FQHC network serves several Vermont towns, offering specialties like cardiology, neurology, OB/GYN, oncology, orthopedics, and surgery. Services also include an Adult Day Program, 49-unit independent living, and a 30-bed nursing home. Gifford Medical Center, a CAH, is home to Vermont’s first alternative birthing center, founded in 1977, and remains a leader in midwifery and family-centered care.
Northern Tier Center for Health is a mission driven primary care practice with a focus on quality and continuity of care. We are a nonprofit corporation governed by a community-based Board of Directors whose mission is to provide high-quality care in Northwestern Vermont. NOTCH’s team of compassionate and devoted health care professionals and support staff develop long-term relationships with patients, often serving multi-generational families. The organization offers a full spectrum of health care services, including family medicine, internal medicine, behavioral health, pharmacy, lab, and general dentistry.
Lamoille Home Health & Hospice is a non-profit home health and visiting nurses agency serving Lamoille County, Vermont, providing home health and hospice care including skilled nursing, rehabilitative therapies, wellness services, telemedicine and long-term care. Their mission is to support health, independence and dignity through quality care in the home. The agency believes that the highest standards of health and end-of-life care are fundamental rights.
Northern Counties Health Care, Inc. serves over 20,000 people—more than a third of Vermont’s Northeast Kingdom—through a network of seven health centers, including walk-in primary care, dental practices, and home health and hospice services. With over 64,000 annual encounters across their rural network, NCHC partners with critical access hospitals and human service groups. As Vermont’s first FQHC, NCHC has led innovations like integrating oral health, piloting the Blueprint for Health, and becoming the state’s first certified Patient Centered Medical Home.
